What is Navicular Drop Test?
The Navicular Drop Test (NDT) is a simple clinical examination used to assess the amount of movement of the navicular bone as the foot changes from a neutral, non-weight-bearing position to a relaxed standing position. It is one of the most commonly performed tests in podiatry, physical therapy, sports medicine, and orthopedic practice for evaluating the function of the medial longitudinal arch of the foot. The test helps determine whether the foot demonstrates excessive pronation, which may contribute to lower-limb biomechanical problems. Since excessive foot pronation can influence ankle, knee, hip, and lower back mechanics, the Navicular Drop Test is frequently included in comprehensive musculoskeletal examinations of athletes and individuals with chronic lower-extremity pain.

The test is inexpensive, non-invasive, and requires only basic equipment such as an index card, ruler, or digital caliper. It is commonly performed in patients with plantar fasciitis, posterior tibial tendon dysfunction, shin splints, patellofemoral pain syndrome, Achilles tendinopathy, and recurrent ankle sprains. Although the Navicular Drop Test provides valuable clinical information about foot posture, it should not be interpreted in isolation. Instead, healthcare professionals combine its findings with gait analysis, range-of-motion testing, muscle strength assessment, and patient symptoms to obtain a complete understanding of foot biomechanics. When used appropriately, it helps identify abnormal foot mechanics and guides treatment decisions such as orthotics, footwear modification, strengthening exercises, or rehabilitation programs.
Procedure
The Navicular Drop Test is performed by first positioning the patient in a seated position with both feet resting comfortably on the floor but without full weight bearing. The examiner identifies and marks the most prominent point of the navicular tuberosity using a skin-safe marker. The subtalar joint is then placed into its neutral position by palpating the talar head while gently moving the foot until equal prominence is felt on both sides. The vertical distance from the floor to the marked navicular tuberosity is measured using a ruler, digital caliper, or measuring card. This measurement represents the neutral arch height before weight bearing.
Next, the patient is instructed to stand naturally with equal weight distributed between both feet while avoiding excessive leaning or shifting. Without changing the marked location, the examiner measures the height of the navicular tuberosity again. The difference between the seated neutral measurement and the standing relaxed measurement is called the navicular drop. The measurement is usually recorded in millimeters (mm). To improve consistency, many clinicians perform the test two or three times and calculate the average value. Accurate patient positioning, consistent subtalar neutral placement, and careful measurement techniques are essential for minimizing measurement error and improving the usefulness of the test during clinical evaluation.
Normal Values
Normal values for the Navicular Drop Test vary slightly among published studies because measurement techniques and patient populations differ. However, in healthy adults, the navicular drop generally ranges from approximately 5 to 9 millimeters. This amount of movement represents normal flexibility of the medial longitudinal arch as body weight is transferred onto the foot during standing. Small variations may occur depending on age, body weight, athletic activity, ligament laxity, and foot structure. Children often demonstrate greater navicular movement because their arches are still developing, while older adults may show reduced flexibility due to degenerative changes or stiffness.
Many clinicians consider a navicular drop of 10 mm or greater to indicate excessive pronation and abnormal foot mechanics. Conversely, very small measurements, such as less than 5 mm, may suggest a rigid foot with limited pronation, which may also predispose an individual to certain injuries. These values should always be interpreted alongside the patient's symptoms, gait pattern, and physical examination findings rather than being used as an isolated diagnostic criterion. Normal ranges provide helpful reference points but should not replace comprehensive clinical judgment when evaluating patients with foot pain or biomechanical abnormalities.
Positive Results
A positive Navicular Drop Test generally refers to excessive lowering of the navicular bone during weight bearing, indicating increased foot pronation. Most clinicians use a cutoff value of approximately 10 mm or more to define a positive result, although some research studies use thresholds between 8 and 10 millimeters depending on the patient population. Excessive navicular movement suggests that the medial longitudinal arch collapses more than expected during standing, potentially placing increased stress on muscles, ligaments, tendons, and joints throughout the lower extremity. Patients with positive findings frequently report foot fatigue, arch pain, heel pain, or discomfort during prolonged standing and walking.
A positive result does not automatically diagnose a specific medical condition. Instead, it indicates abnormal foot biomechanics that may contribute to disorders such as plantar fasciitis, posterior tibial tendon dysfunction, medial tibial stress syndrome (shin splints), Achilles tendinopathy, patellofemoral pain syndrome, iliotibial band syndrome, or recurrent ankle sprains. Following a positive test, clinicians usually perform additional examinations including gait analysis, footwear assessment, muscle strength testing, ankle dorsiflexion measurements, and imaging studies when appropriate. Treatment recommendations may include strengthening exercises, foot orthoses, activity modification, taping techniques, stretching programs, or referral to a podiatrist or physical therapist.
Flat Foot Interpretation
The Navicular Drop Test plays an important role in evaluating individuals with pes planus (flat feet). In flexible flatfoot, the medial longitudinal arch appears normal while sitting but collapses significantly during standing. This dynamic collapse results in an increased navicular drop measurement and excessive pronation. Patients with flexible flat feet often experience foot fatigue, arch discomfort, heel pain, reduced athletic performance, or recurrent lower-limb injuries. The Navicular Drop Test helps clinicians quantify the severity of arch collapse and monitor improvement after rehabilitation or orthotic intervention.
In contrast, patients with rigid flatfoot may demonstrate relatively little navicular movement despite having a low arch because the foot lacks normal flexibility. Therefore, interpretation of the Navicular Drop Test must consider overall foot structure and mobility. A high navicular drop suggests flexible pronation, whereas minimal movement in the presence of a flattened arch may indicate structural rigidity requiring different treatment strategies. Clinicians often combine the Navicular Drop Test with the Foot Posture Index (FPI), Jack's Test, rearfoot alignment assessment, and walking gait analysis to distinguish between flexible and rigid flatfoot deformities and develop individualized treatment plans.
Reliability
Numerous clinical studies have evaluated the reliability of the Navicular Drop Test. Overall, the test demonstrates moderate to good intra-rater reliability, meaning the same examiner can usually obtain consistent measurements when repeating the procedure under standardized conditions. However, inter-rater reliability is generally lower because identifying the subtalar neutral position and locating the navicular tuberosity may vary between clinicians. Small differences in patient positioning, measurement tools, examiner experience, and anatomical landmarks can influence results by several millimeters. Standardized protocols and examiner training significantly improve measurement consistency.
Although the Navicular Drop Test remains one of the most widely used assessments for dynamic foot posture, it should not be considered a standalone diagnostic tool. Modern clinical practice recommends combining it with functional movement assessments, gait analysis, imaging when indicated, and patient-reported symptoms to improve diagnostic accuracy. Despite its limitations, the Navicular Drop Test continues to be valued because it is quick, inexpensive, requires minimal equipment, and provides clinically meaningful information regarding foot biomechanics. When performed correctly by trained clinicians, it remains a reliable component of lower-extremity musculoskeletal assessment and rehabilitation planning.
