Chocolate Cyst - Size, Rupture, Discharge, Ultrasound

Chocolate cyst is also known as endometrioma and endometrium cyst. A growth appears in the ovary which is filled with blood known as chocolate cyst. It may be located on the surface of ovary or may present inside the ovary i.e. ovary encapsulate the cyst. These blood filled cyst may be larger in size. The main cause seen behind chocolate cyst is the retrograde menstruation. In this condition (chocolate cyst) the menstrual blood start flowing backwards toward ovary and start accumulating there as a result endometrial cyst may form inside and may not be detected until leakage of blood occurs from ovaries.

The size of endometrioma or chocolate cyst or endometrium cyst may vary in size according to worsening of condition. The largest size of chocolate cyst reported is 20 cm and the smallest size reported is 2 cm. chocolate cyst normally occurs in size ranging 2 to 5 cm. chocolate cyst may appear 1.5 i.e. size of grape to 4 inches i.e. size of softball in diameter. Increased size of chocolate cyst shows the worsening of condition and the treatment is also difficult. The increased size of chocolate cyst can cause blood overflow from ovary.

Chocolate Cyst Rupture

           
The largest size of cyst may got rupture and burst. When the chocolate cyst rupture, a sudden pain occurs in the lower abdomen at the site of ovary. Internal bleeding occurs as a result of cyst rupture. Most of the chocolate cyst when burst, show no symptoms or very less symptoms i.e. intense sudden pain. But in some cases, severe symptoms may appear. Chocolate cyst may get rupture during intercourse and cause severe bleeding along with pain in lower abdominal pain and nausea and vomiting. If it get burst during intercourse, then sudden sharp pain will occur along with bleeding.

The chocolate cyst when rupture or burst due to inappropriate intercourse and vigorous exercise the bloody discharge occurs through vagina. Along with discharge the women may also feel tachycardia, dizziness and fever. In the rupture of chocolate cyst, the chocolate colored reddish discharge occurs. The discharge occurs may be heavy or minute spotting. This discharge occurs just before the periods in the form of spots and the women may consider it as irregular periods. The blood from the cyst i.e. black in color will mix up with other vaginal discharge and appear as brown color vaginal discharge.

The most common method of diagnosis is the ultrasound of pelvic region. Magnetic resonance imaging can also be used for diagnosing as it gives better results as compare to ultrasound scanning. Ultrasound is not active against the diagnosis of upper layer lesions and may also not detect the large implants. Ultrasound may not show 100 % diagnosis all the time that’s why magnetic resonance imaging MRI is done to get the exact diagnosis. Mostly large sized chocolate cysts can be diagnosed with ultrasound and the small sized chocolate cysts are not detected.
