What is MARPE Expander?
MARPE stands for Miniscrew-Assisted Rapid Palatal Expansion. A MARPE expander is an orthodontic appliance designed to widen a narrow upper jaw by transferring expansion forces through small orthodontic miniscrews anchored in the palate. Traditional rapid palatal expanders mainly rely on the teeth for anchorage, whereas MARPE is designed to place a greater portion of the expansion force into the palatal bone. This makes it particularly useful for older adolescents and selected adults who have a transverse maxillary deficiency or a narrow upper jaw. The objective is to obtain more skeletal expansion and reduce unwanted dental tipping compared with conventional tooth-supported expansion. Research indicates that MARPE can produce meaningful skeletal expansion in late adolescents and adults, although treatment success varies considerably with age, skeletal maturity, anatomy, appliance design, and clinical technique.
A MARPE appliance generally consists of an expansion screw connected to a framework that is stabilized by several miniscrews placed in the palate. Before treatment, an orthodontist evaluates the width of the maxilla, bite relationship, periodontal condition, palatal anatomy, and skeletal maturity. Three-dimensional imaging such as CBCT may be used when necessary to assess available bone and plan miniscrew placement. The appliance is then activated according to an individualized orthodontic protocol. Patients may notice pressure or temporary discomfort as expansion occurs. A space between the upper front teeth can develop when the midpalatal suture opens, although the appearance and amount of expansion vary between patients. Unlike a simple removable orthodontic appliance, MARPE involves customized appliance fabrication and placement of temporary skeletal anchorage devices, so the total treatment fee can be higher than conventional palatal expansion. Some clinics quote the expander separately, while others include consultations, CBCT imaging, appliance fabrication, miniscrew placement, adjustments, follow-up visits, and subsequent orthodontic treatment in one package. Therefore, an advertised appliance price should not automatically be considered the complete treatment cost.

Evidence reviews describe MARPE as having lower biological and financial costs than some surgical approaches, while also emphasizing that treatment selection must account for skeletal maturity and the possibility of expansion failure.
MARPE Expander for Adults
MARPE for adults is particularly relevant because conventional rapid palatal expansion becomes less predictable as the midpalatal suture and surrounding structures mature. In younger patients, the maxilla responds more readily to orthopedic expansion. With increasing age, the sutures become more interdigitated and resistant, making purely tooth-supported expansion more likely to produce dental movement rather than the desired skeletal change. MARPE attempts to overcome this limitation by anchoring the appliance directly to the palatal bone. Studies in late adolescents and adults have reported substantial skeletal expansion, with one systematic review and meta-analysis reporting a pooled mean success rate of approximately 92.5%; however, the authors rated the overall evidence quality as very low because many included studies had significant risk of bias.
Adult treatment therefore requires careful case selection rather than assuming that MARPE will work simply because a patient is an adult. Age, skeletal maturity, palatal bone anatomy, transverse deficiency, periodontal health, and the resistance of the craniofacial structures all influence the likelihood of success. Some adults may experience incomplete or asymmetric expansion, miniscrew problems, dental tipping, periodontal changes, or failure of the intended skeletal opening. A recent adult-focused review found that expansion failure and dentoalveolar effects remain important considerations and emphasized that patient age and treatment planning influence the choice between MARPE and surgically assisted expansion.
MARPE Expander in Orthodontics
In orthodontics, MARPE is primarily used to manage transverse maxillary deficiency, which may appear clinically as a narrow upper dental arch, posterior crossbite, or an inadequate relationship between the upper and lower jaws. The treatment goal is to increase maxillary transverse width while obtaining as much skeletal correction as possible and limiting unnecessary movement of the supporting teeth. Once expansion is achieved and stabilized, additional orthodontic treatment may be required to align the teeth and establish a stable bite. MARPE can therefore be one stage of comprehensive orthodontic treatment rather than a complete treatment by itself.
MARPE is also different from SARPE, or surgically assisted rapid palatal expansion. MARPE attempts to achieve skeletal expansion without conventional surgical assistance, whereas SARPE uses a surgical procedure to reduce resistance to expansion in appropriately selected patients. MARPE may offer less surgical morbidity, but it does not eliminate complications or guarantee success. Reported issues include miniscrew loosening or failure, soft-tissue irritation, dental tipping, periodontal changes, asymmetric expansion, and failure to achieve the intended skeletal separation. Recent literature also emphasizes that careful diagnosis and appliance positioning are important, particularly in mature adults. Consequently, orthodontists may use clinical examination and CBCT-based assessment to determine whether MARPE, conventional expansion, or a surgical approach provides the most predictable solution.
MARPE Expander MRI Safety
MRI safety with a MARPE expander depends on the exact appliance, its metals, its manufacturer specifications, and whether the device has been evaluated for the particular MRI environment. A MARPE appliance may contain stainless steel, titanium, or other metallic components, including miniscrews. The presence of metal does not automatically mean that MRI is prohibited, but the device should not simply be assumed to be MRI-safe. Patients should tell the radiology department about the MARPE appliance and provide the appliance's manufacturer information whenever possible. The MRI facility can then determine whether the specific device is MR Safe, MR Conditional, or unsuitable for the planned examination.
Another consideration is image artifact. Even when an orthodontic appliance can safely remain in place, metallic components may produce distortion or signal loss near the mouth, face, jaw, or skull. This can be particularly relevant for MRI examinations of the head and neck. Do not remove miniscrews or an orthodontic appliance solely for an MRI unless the orthodontist and radiology team specifically recommend it. Before an MRI appointment, provide the radiologist or MRI technologist with the exact appliance details and ask whether any special conditions apply to the scanner's magnetic field strength and sequence. MRI safety should therefore be determined on a device-specific basis rather than from a generic statement that all MARPE appliances are safe or unsafe.
