Endopyelotomy is a successful technique for individuals with obstruction of the ureteropelvic junction. An obstruction at the ureteropelvic junction (also known as UPJ obstruction) is a blockage that occurs in the area that joins the renal pelvis to one of the tubes (ureters) that carry urine to the bladder. Obstruction of the ureteropelvic junction (UPJ) can be induced by congenital anomalies such as fibrous scarring due to a kidney stone or a prior surgical treatment, a blood artery that may coil the UPJ, or an embedded stone in the upper portion of the ureter. This may result in kidney tissue injury, stone development, infection, elevated blood pressure, worsening of kidney function, and ultimately kidney failure. Endopyelotomy is a common surgery used to address these problems. Patients who undergo percutaneous endopyelotomy can anticipate a success rate of up to 90%, minimal morbidity, and minor disability.

Definition

Endopyelotomy is a minimally invasive surgery method that has proven to be effective in the treatment of obstruction at the ureteropelvic junction (UPJ). When there is a physical or anatomical obstacle to the passage of urine from the renal pelvis into the ureter, this is known as ureteropelvic junction obstruction (UPJO).

Procedure

Endopyelotomy is a surgery in which a telescope or balloon containing an electric wire is inserted into the kidney through the ureter. This treatment is performed to remove kidney stones. After that, an incision is made into the middle of the scar tissue. In contrast, to open surgery, these procedures can be completed as an outpatient in a short period of time with little anesthesia and a considerably shorter recovery period. A temporary internal tube (stent) must be kept in place by the patient for 4 to 6 weeks. These methods have a radiographic success rate that is between 15 to 20 % lower than that is acquired by open surgery.

In order to achieve the same high success rate as open pyeloplasty while reducing morbidity, laparoscopic pyeloplasty was created. Without requiring a major incision, the inside treatment is carried out in the same way as open surgery. Indications

When the ureter is blocked, it can cause pain in the flank on the side that is affected. Some people report that their pain gets worse when they drink more alcohol, coffee, or drinks. The discomfort may also come and go. Pain can occasionally be felt at the front of the abdomen, and it may spread all the way down to the groin. Sometimes, while evaluating a patient for other issues, the disease is unintentionally found on x-rays or ultrasounds. Patients may experience severe illness and high fevers when infection and blockage coexist. This condition may require hospitalization and surgical intervention of obstruction.
